New Warning Labels

Warning Signs Production In 2019, the Food and Drug Administration reported its preliminary results from the Xeljanz clinical trials, which highlighted an elevated risk of blood clots and death. Specifically, the federal agency reported 19 cases of blood clots and 45 fatalities in patients who took 10-milligrams of Xeljanz compared to only 3 cases of blood clots and 25 deaths in those who received tumor necrosis factor (TNF) blockers. Consequently, the Food and Drug Administration required Pfizer to add a black box warning to its Xeljanz labels. Before the placement of this black box warning was added in July 2019, the pharmaceutical company failed to warn patients about the risk of blood clots.

On February 4, 2021, after Xeljanz failed to meet the endpoint criteria in a clinical trial for major adverse cardiovascular events, the Food and Drug Administration issued an FDA Drug Safety Communication regarding the drug’s increased risk of serious heart-related events, cancer, blood clots, and death. On September 1, 2021, the agency ordered a revision to Xeljanz’s warning label to caution patients about the increased risks of potentially fatal conditions, such as heart attack, stroke, cancer, and blood clots. According to the Food and Drug Administration’s latest review of the clinical data, Xeljanz users exhibited dramatically higher rates of lung cancer and lymphoma compared to the control groups. Although a future recall is looming, as of today, Xeljanz has not been recalled. However, the agency has restricted all approved uses of the drug to patients who have been unresponsive or cannot tolerate tumor necrosis factor blockers to ensure that the benefits of Xeljanz outweigh the substantial risks.
